Who is This Program For?
This IOP isn’t for everyone but it is a game-changer for people who:
- Just left a hospital or inpatient program and need follow-up care
- Keep hitting a crisis point again and again
- Feel like daily life is just too much to manage right now
- Need more than weekly sessions to stay on track
If any of that sounds like you, or someone you care about, let’s talk. You don’t have to be “in crisis” to benefit from extra support. Sometimes just getting through the week is enough reason to reach out.
What Happens in the IOP?
Let’s break it down, here’s what our IOP actually includes:
- Three small-group therapy sessions each week (with folks who get it)
- One-on-one therapy with a licensed clinician
- Medication management with a psychiatrist (no pressure, just options)
- Personalized treatment planning that focuses on you
- Optional family and educational groups
We offer both in-person care at our Santa Rosa office and a virtual IOP for people who live farther out or can’t always make it in. Flexibility matters.
Conditions We Work With
Our team works with a wide range of mental health conditions. You don’t have to fit into a perfect box. Some of the most common issues we help with include:
- Major depression (including the kind that doesn’t respond to meds)
- Anxiety disorders
- Bipolar and other mood disorders
- PTSD and trauma recovery
- Dual diagnosis / co-occurring substance use and mental health challenges
Not sure if your experience “qualifies”? Just ask. We’ll walk through it with you.
Real Therapy. Real Skills. Real Progress.
Group therapy is a big part of our IOP and we promise, it’s not awkward. Our groups are built to help you actually get something out of it, not just talk in circles.
Some of the tools and topics we cover include:
- Learning how to deal with stress and emotional overwhelm
- DBT skills (yep, that’s the mindfulness and emotion-regulation stuff)
- CBT strategies to help you shift unhelpful thinking patterns
- Relapse prevention (for both mental health and substance use)
- Better communication and relationship tools
- Nutrition, sleep, and medication info that actually matters
Plus, we offer evening IOP options if you work during the day. Because your life doesn’t pause for treatment.
Can’t Make It In-Person? You’ve Got Options.
Our Virtual IOP (aka telehealth) brings the same level of care straight to your screen. All sessions happen through secure video calls, so you can participate from the comfort of home. It’s perfect for:
- People in more rural parts of Sonoma County
- Clients with transportation or mobility issues
- Anyone balancing family or work responsibilities
You’ll still meet with real people, not just log into a portal. The virtual version lets you stay connected without losing access to high-quality care.
Why Siyan Clinical?
Fair question. Here’s what sets us apart:
- We’re local. Based right here in Santa Rosa and serving all of Sonoma County.

- Our team includes board-certified psychiatrists, licensed therapists, and support staff who actually care.

We know this isn’t easy. Mental health can be complicated. But we’re here for the messy parts, not just the milestones.
