At Siyan Clinical in Santa Rosa, we offer a structured but flexible program that gives you serious support, without disrupting your entire life.

Who is This Program For?

This IOP isn’t for everyone but it is a game-changer for people who:

  • Just left a hospital or inpatient program and need follow-up care
  • Keep hitting a crisis point again and again
  • Feel like daily life is just too much to manage right now
  • Need more than weekly sessions to stay on track

If any of that sounds like you, or someone you care about, let’s talk. You don’t have to be “in crisis” to benefit from extra support. Sometimes just getting through the week is enough reason to reach out.

What Happens in the IOP?

Let’s break it down, here’s what our IOP actually includes:

  • Three small-group therapy sessions each week (with folks who get it)
  • One-on-one therapy with a licensed clinician
  • Medication management with a psychiatrist (no pressure, just options)
  • Personalized treatment planning that focuses on you
  • Optional family and educational groups

We offer both in-person care at our Santa Rosa office and a virtual IOP for people who live farther out or can’t always make it in. Flexibility matters.

Conditions We Work With

Our team works with a wide range of mental health conditions. You don’t have to fit into a perfect box. Some of the most common issues we help with include:

  • Major depression (including the kind that doesn’t respond to meds)
  • Anxiety disorders
  • Bipolar and other mood disorders
  • PTSD and trauma recovery
  • Dual diagnosis / co-occurring substance use and mental health challenges

Not sure if your experience “qualifies”? Just ask. We’ll walk through it with you.

Real Therapy. Real Skills. Real Progress.

Group therapy is a big part of our IOP and we promise, it’s not awkward. Our groups are built to help you actually get something out of it, not just talk in circles.

Some of the tools and topics we cover include:

  • Learning how to deal with stress and emotional overwhelm
  • DBT skills (yep, that’s the mindfulness and emotion-regulation stuff)
  • CBT strategies to help you shift unhelpful thinking patterns
  • Relapse prevention (for both mental health and substance use)
  • Better communication and relationship tools
  • Nutrition, sleep, and medication info that actually matters

Plus, we offer evening IOP options if you work during the day. Because your life doesn’t pause for treatment.

Can’t Make It In-Person? You’ve Got Options.

Our Virtual IOP (aka telehealth) brings the same level of care straight to your screen. All sessions happen through secure video calls, so you can participate from the comfort of home. It’s perfect for:

  • People in more rural parts of Sonoma County
  • Clients with transportation or mobility issues
  • Anyone balancing family or work responsibilities

You’ll still meet with real people, not just log into a portal. The virtual version lets you stay connected without losing access to high-quality care.

Why Siyan Clinical?

Fair question. Here’s what sets us apart:

  • We’re local. Based right here in Santa Rosa and serving all of Sonoma County.
  • We treat you like a person, not a chart. No rushed care or one-size-fits-all plans.
  • Our team includes board-certified psychiatrists, licensed therapists, and support staff who actually care.
  • We’ve helped hundreds of adults stabilize, rebuild, and move forward.

We know this isn’t easy. Mental health can be complicated. But we’re here for the messy parts, not just the milestones.

FREQUENTLY ASKED QUESTIONS

What is an intensive outpatient program (IOP)?

An Intensive Outpatient Program (IOP) is a structured mental health treatment program designed for individuals who need more support than traditional individual therapy but do not require 24-hour residential care. IOPs offer a focused and accelerated approach to addressing various mental health conditions, including depression, anxiety, trauma, PTSD, and other significant challenges.

Key Features of IOPs:

  • Short-Term, Intensive Treatment: With sessions several days a week, IOPs allowing for concentrated progress in a shorter time frame.
  • Group Therapy Emphasis: Group therapy is a central component of IOPs, providing a supportive environment where individuals can share experiences, learn from others, and develop coping skills.
  • Individual Therapy: IOPs also include individual therapy sessions to address specific needs and tailor treatment to each person’s unique circumstances.
  • Escalated Level of Care: IOPs offer a higher level of support than standard outpatient therapy, making them suitable for those who have not found sufficient relief with less intensive options.

How does the IOP process work?

First, your care team must determine whether an IOP is a good fit for you at this stage. If it is, your IOP team (consisting of a therapist, psychiatrist, care coordinator, and specialists when appropriate), will create a personalized plan outlining your goals and the therapies involved. Group therapy is at the center of the IOP program, with three sessions a week (each lasting three hours). Throughout the program, the team monitors your progress and adjusts your treatment plan as needed. As you reach your goals, they help you create an aftercare plan to support your continued recovery after leaving the IOP.

How does IOP compare to inpatient care?

Intensive Outpatient Programs (IOPs) and inpatient care are both designed to address mental health challenges, but they differ significantly in intensity and structure.

Intensity: IOPs are less intensive than inpatient hospitalization or partial hospitalization, offering a step down from 24/7 care while still providing a structured treatment environment. This makes IOPs suitable for individuals who have a safe and stable living situation but require more support than traditional outpatient therapy.

Structure: IOPs typically involve 9+ hours of treatment per week, spread across several days. Treatment often includes a combination of group therapy, individual therapy, and other modalities like medication management.

Delivery: IOPs can be delivered in person or through telehealth, offering flexibility and convenience for individuals who may not be able to attend a physical facility.

Do I need to take medications with IOP?

Medications are not required but may be helpful in some cases. During your initial evaluation, you will discuss your situation and needs with a psychiatric provider.

What types of mental health challenges are addressed in an IOP?

IOPs are designed to help individuals facing a wide range of mental health challenges, including but not limited to:

  • Depression
  • Anxiety disorders
  • Mood disorders (such as bipolar)
  • Co-occurring psychiatric disorders
  • Substance abuse

If you’re wondering whether our IOP program is suitable for your needs, please reach out for a consultation.

What is the schedule for IOP at Siyan?

Our IOP includes group therapy sessions each Tuesday, Wednesday, and Thursday from 9 a.m. to 1:30 p.m. Weekly individual therapy sessions are scheduled separately.

Will my insurance cover the IOP program?

Many insurance policies cover intensive outpatient programs, but your coverage will depend on your individual insurance plan. Eligibility is determined prior to admission to the program.

Where does Siyan’s IOP take place?

You may choose to attend the IOP in person at our comfortable, convenient Siyan Care Campus in Santa Rosa, or virtually from your own home with our telehealth option.

What is a telehealth visit?

A telehealth visit is an audio-visual, real-time exchange (a video call) between you and a provider that takes place instead of a scheduled in-person appointment. These types of visits are also sometimes referred to as telemedicine visits.

Where do telehealth visits take place?

With telehealth, you can have your visit from the comfort and safety of your own home via a computer, tablet or cell phone with video capability and an internet connection. The provider will connect with you via the internet.

Do I need special hardware or software for my telehealth visits?

We use a secure telehealth platform called Tebra for our telehealth appointments. You will not need to download any software or application prior to your visit. You will receive a link for your appointment by email prior to your scheduled appointment.

You may use your mobile phone (iOS or Android), computer, or tablet. The device must be able to make phone calls and have video capabilities, a microphone, and audio. You also must have a reliable, high-speed internet connection ( WiFi or a cellular data connection with 5G LTE). Make sure your internet is stable and it can run video without a break.

On the day of your appointment, please make sure your device is sufficiently charged or plugged in; you can log in to your visit up to 5 minutes prior to the start time.

Why Should I use Telehealth?

It is not always easy or convenient for patients to get help for their healthcare needs. Telehealth offers an alternative way to see providers and get the healthcare you need without leaving home. Telehealth reduces the time you spend traveling to a clinic and waiting for your appointment, as well as your time away from work and other priorities.

Does my insurance cover telehealth?

Coverage for services is dependent upon each individual client’s insurance plan. Eligibility is determined prior to admission.
